Common use
Ventolin HFA is primarily indicated for the treatment and prevention of bronchospasm in patients 4 years of age and older with reversible obstructive airway disease, such as asthma. It is also approved for the prevention of exercise-induced bronchospasm (EIB). It is classified as a rescue medication and is not intended for regular daily use or as a monotherapy for asthma. Its role is to provide immediate, as-needed relief from acute symptoms.
Dosage and direction
The dosage of Ventolin HFA must be individualized based on patient need and response. It is crucial to follow the prescribing physician’s instructions exactly.
- For treatment of acute bronchospasm: The usual dose for adults and children aged 4 years and older is 2 inhalations every 4 to 6 hours as needed.
- For prevention of exercise-induced bronchospasm (EIB): The usual dose is 2 inhalations 15 to 30 minutes before exercise.
- Administration Technique:
- Shake the inhaler well before each use.
- Breathe out fully, away from the inhaler.
- Place the mouthpiece in your mouth and close your lips around it, or hold it 1-2 inches from your open mouth (or use a spacer).
- Start to breathe in slowly and deeply, and at the same time, press down firmly on the canister to release the medication.
- Continue to breathe in slowly for 3-5 seconds.
- Hold your breath for 10 seconds to allow the medication to settle in the airways.
- Wait at least 30-60 seconds before taking a second puff, if prescribed.
- Note: The canister must be primed before first use and if not used for more than 2 weeks. Rinsing your mouth with water after use is recommended to reduce the risk of oropharyngeal side effects.
Precautions
- Ventolin HFA can produce significant cardiovascular effects such as increased heart rate, blood pressure, and palpitations. Use with extreme caution in patients with cardiovascular disorders (e.g., coronary insufficiency, cardiac arrhythmias, hypertension).
- Paradoxical bronchospasm (a sudden worsening of breathing) can occur immediately after inhalation. If this happens, discontinue use immediately and institute alternative therapy.
- Do not increase the dose or frequency of use without consulting a physician. Overuse indicates deteriorating asthma control and necessitates a re-evaluation of the patient’s maintenance therapy regimen.
- Use with caution in patients with convulsive disorders, hyperthyroidism, diabetes, and in patients unusually responsive to sympathomimetic amines.
- Potentially serious hypokalemia may result from beta-agonist therapy, particularly in combination with xanthine derivatives, steroids, or diuretics. This effect may be potentiated by hypoxia.
Contraindications
Ventolin HFA is contraindicated in patients with a history of hypersensitivity to albuterol or any other component of the formulation (including the HFA propellant).
Possible side effect
Like all medications, Ventolin HFA can cause side effects, although not everybody gets them.
- Very Common (>10%): Headache, nervousness, tremor.
- Common (1-10%): Tachycardia (fast heart rate), palpitations, throat irritation, muscle cramps, cough, dizziness.
- Uncommon (0.1-1%): Hypersensitivity reactions (e.g., urticaria, rash, bronchospasm), hypokalemia.
- Rare (<0.1%): Angina, hypertension, hypotension, paradoxical bronchospasm, sleep disturbance, hyperactivity.
- Patients should be instructed to report any unusual or severe side effects to their healthcare provider immediately.
Drug interaction
Concomitant use of Ventolin HFA with other drugs can potentiate the risk of adverse effects.
- Beta-blockers: Non-selective beta-blockers (e.g., propranolol) can antagonize the bronchodilator effect of albuterol and produce severe bronchospasm in asthmatic patients. Use with caution; cardioselective beta-blockers are preferred if needed.
- Diuretics and xanthine derivatives: May potentiate the ECG changes and/or hypokalemia that can be caused by albuterol. Monitoring of potassium levels is advised.
- Monoamine oxidase inhibitors (MAOIs) and Tricyclic antidepressants (TCAs): Can potentiate the cardiovascular effects of albuterol. Use with extreme caution.
- Other sympathomimetic agents: Concomitant use may lead to additive cardiovascular effects and should be avoided unless deemed necessary by a physician.
Missed dose
Ventolin HFA is used on an as-needed basis for symptom relief. It is not intended for scheduled dosing, so the concept of a “missed dose” does not typically apply. If it is being used prophylactically for EIB and a dose is missed, it may be taken as soon as remembered if there is still sufficient time before exercise. Do not double the dose to make up for a missed one.
Overdose
Overdosage with albuterol is likely to produce exaggeration of its known pharmacologic effects and adverse reactions.
- Symptoms: May include seizures, angina, hypertension or hypotension, tachycardia with rates up to 200 beats per minute, nervousness, headache, tremor, dry mouth, palpitation, nausea, dizziness, fatigue, malaise, hypokalemia, and hyperglycemia.
- Treatment: Discontinuation of Ventolin HFA and institution of appropriate supportive and symptomatic therapy is indicated. Cardioselective beta-blockers can be considered but should be used with caution as they may induce bronchospasm. Monitoring of cardiac rhythm and serum potassium is essential.
Storage
- Store at controlled room temperature between 20°C and 25°C (68°F and 77°F). Do not freeze.
- Keep the canister away from direct sunlight, heat, and open flame. Exposure to temperatures above 120°F may cause bursting.
- The contents of the canister are under pressure. Do not puncture, break, or incinerate the canister, even after it is apparently empty.
- Keep out of reach of children and pets.
