Tylenol (acetaminophen) is a leading over-the-counter analgesic and antipyretic medication with a well-established safety profile when used as directed. As a non-aspirin pain reliever, it provides effective temporary relief from minor aches and pains associated with headaches, muscle aches, arthritis, backaches, toothaches, the common cold, and menstrual cramps. It also effectively reduces fever. Unlike NSAIDs, Tylenol is not associated with stomach irritation or bleeding, making it a suitable option for many patients. This monograph provides comprehensive, expert-level information for healthcare professionals and informed consumers.
Features
- Active ingredient: Acetaminophen (500 mg per caplet/tablet in regular strength formulations)
- Mechanism of action: Central cyclooxygenase inhibition with primarily antipyretic and analgesic effects
- Formulations: Caplets, tablets, gelcaps, geltabs, liquid, and dissolve packs
- Non-prescription status (OTC) in standard strength doses
- Rapid absorption with peak plasma concentrations occurring within 30-60 minutes
- Hepatic metabolism primarily via glucuronidation and sulfation pathways
Benefits
- Provides effective temporary relief from mild to moderate pain without gastrointestinal irritation common with NSAIDs
- Rapidly reduces fever through action on the hypothalamic heat-regulating center
- Suitable for patients with aspirin sensitivity, NSAID allergies, or bleeding disorders
- Does not interfere with platelet aggregation or prolong bleeding time
- Available in multiple formulations for personalized dosing and administration preferences
- Established safety profile across diverse patient populations when used appropriately
Common use
Tylenol is indicated for the temporary relief of minor aches and pains associated with: headaches, muscular aches, backaches, minor arthritis pain, toothaches, menstrual cramps, and the common cold. It is also indicated for the temporary reduction of fever. The medication works by elevating the pain threshold and acting on the hypothalamic heat-regulating center to produce antipyresis. It is particularly valuable for patients who cannot tolerate NSAIDs or for whom anti-inflammatory effects are not required.
Dosage and direction
Adults and children 12 years and older: Take 2 caplets (1000 mg) every 4-6 hours while symptoms persist. Do not take more than 6 caplets (3000 mg) in 24 hours, unless directed by a physician. The maximum daily dose should not exceed 4000 mg for most adults.
Children 6-11 years: Consult pediatric dosing guidelines based on weight and age. Typically 10-15 mg/kg per dose every 4-6 hours, not to exceed 5 doses in 24 hours.
Administration: Swallow whole with water. May be taken with or without food. Do not crush, chew, or dissolve extended-release formulations. For liquid formulations, use the measuring device provided.
Precautions
Do not use with other products containing acetaminophen. Patients with hepatic impairment should use with caution and consult a healthcare provider. Chronic alcohol users should not exceed 2000 mg daily. Do not use for more than 10 days for pain or 3 days for fever unless directed by a physician. Discontinue use and consult a doctor if pain worsens or persists beyond 10 days, fever persists beyond 3 days, or new symptoms occur. Keep out of reach of children.
Contraindications
Hypersensitivity to acetaminophen or any component of the formulation. Severe hepatic impairment or active liver disease. Should not be used in patients with severe renal impairment (CrCl <30 mL/min) without medical supervision. Not recommended for use in children under 2 years of age without consulting a healthcare provider.
Possible side effects
In recommended doses, acetaminophen is generally well-tolerated. Rare adverse effects may include:
- Allergic reactions (skin rash, urticaria, angioedema)
- Hematologic effects (thrombocytopenia, leukopenia)
- Hepatic enzyme elevation (with excessive doses or pre-existing liver conditions)
- Gastrointestinal discomfort (nausea, epigastric pain)
- Rare cases of serious skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis)
Drug interaction
May interact with warfarin, potentially increasing INR and risk of bleeding (monitor coagulation parameters). Concomitant use with other hepatotoxic agents (including alcohol, anticonvulsants, and antituberculosis drugs) may increase risk of liver damage. Barbiturates, carbamazepine, phenytoin, and rifampin may increase metabolism of acetaminophen. May decrease effectiveness of lamotrigine. Cholestyramine may decrease absorption if taken within 1 hour.
Missed dose
As Tylenol is taken as needed for pain or fever, there is no fixed dosing schedule. If using regularly and a dose is missed, take it as soon as remembered unless it is almost time for the next dose. Do not double doses to make up for a missed dose.
Overdose
Acetaminophen overdose can cause serious, potentially fatal liver damage. Early symptoms may include nausea, vomiting, sweating, and malaise. Clinical evidence of liver damage may not be apparent for 48-72 hours post-ingestion. In case of suspected overdose, seek immediate medical attention even if the patient appears well. Activated charcoal may be effective if administered within 1-2 hours of ingestion. N-acetylcysteine is the specific antidote and is most effective when initiated within 8 hours of overdose.
Storage
Store at room temperature (20-25°C or 68-77°F). Protect from excessive moisture and heat. Keep bottle tightly closed. Do not store in bathroom medicine cabinet due to humidity fluctuations. Keep out of reach of children and pets. Discard expired medication properly according to local guidelines.
