Common use
The Tiova Inhaler is indicated for the long-term, maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD). It is used to manage symptoms such as wheezing, shortness of breath, and chronic coughing. It is not a rescue medication and should not be used to treat sudden attacks of breathlessness.
Dosage and direction
The recommended dosage for adults is the inhalation of the contents of one capsule once daily, at the same time each day, using the Tiova Inhaler device. The capsule must only be used with the designated inhaler and must not be swallowed. Instructions for use:
- Open the dust cap and then the mouthpiece.
- Place the capsule in the capsule chamber.
- Close the mouthpiece firmly until a click is heard.
- Hold the inhaler upright and press the piercing button completely once and release.
- Breathe out fully away from the mouthpiece.
- Place the mouthpiece between your lips and breathe in deeply and steadily at a rate sufficient to hear the capsule vibrate.
- Hold your breath for as long as comfortable, then exhale away from the inhaler.
- Repeat steps 5 and 6 to ensure the full dose is inhaled.
- Open the mouthpiece and discard the empty capsule.
Precautions
- Inform your healthcare provider if you have a history of kidney problems, narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction.
- It may cause dizziness or blurred vision; do not drive or operate machinery until you know how it affects you.
- Immediate hypersensitivity reactions, including angioedema, may occur. Discontinue use and seek medical attention if symptoms appear.
- As with other inhaled medicines, Tiova may cause paradoxical bronchospasm (wheezing immediately after inhalation). If this occurs, discontinue use and use a fast-acting inhaler, then consult your physician.
- Be aware that this therapy treats the symptoms of COPD but does not alter the progressive nature of the disease.
Contraindications
Tiova Inhaler is contraindicated in patients with a known hypersensitivity to tiotropium bromide, atropine or its derivatives, such as ipratropium, or any other component of the product.
Possible side effect
Common side effects may include:

- Constipation
- Increased heart rate (tachycardia)

- Urinary retention Less common but serious side effects require immediate medical attention:
- Worsening of narrow-angle glaucoma (eye pain, blurred vision, visual halos)

- Severe allergic reactions (rash, hives, itching, swelling of the face/lips/tongue)
Drug interaction
- Concomitant use with other anticholinergic-containing drugs (e.g., ipratropium, aclidinium) is not recommended as it may potentiate adverse effects.
- Use caution when co-administering with drugs that also possess anticholinergic properties (e.g., some antidepressants, antihistamines, antipsychotics).
- Inform your doctor of all other medications you are taking, including prescription, over-the-counter, vitamins, and herbal products.
Missed dose
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and resume your usual dosing schedule. Do not take a double dose to make up for a missed one.
Overdose
An overdose may lead to an exacerbation of anticholinergic effects such as dry mouth, visual disturbance, tachycardia, and difficulty in urination. In severe cases, delirium, psychosis, and cardiac arrhythmias may occur. There is no specific antidote. Treatment should be supportive and symptomatic. Medical attention should be sought immediately.
Storage
- Store at room temperature (15°C - 30°C), protected from light and moisture.
- Keep the capsules in the blister pack and only remove immediately before use.
- Always keep the device dry.
- Keep out of reach of children and pets.
