Common use

Symbicort Turbuhaler is regularly prescribed for maintenance treatment of asthma in patients aged 18 years and older, where a combination product is appropriate. It is also indicated for symptomatic treatment of patients with severe COPD (FEV1 < 50% predicted normal) and a history of repeated exacerbations. It is not intended for relief of acute bronchospasm; a separate rescue inhaler (e.g., SABA) should be used for immediate symptom relief.

Dosage and direction

For asthma: The usual maintenance dose is 1โ€“2 inhalations twice daily. Maximum recommended dose is 4 inhalations twice daily. For COPD: The recommended dose is 2 inhalations twice daily. Always inhale deeply and forcefully. Rinse mouth with water after each use to reduce risk of oral candidiasis. Do not exceed prescribed dosage. The Turbuhaler must be held upright during loading and inhalation.

Precautions

Not for initial treatment of asthma; begin under medical supervision. May increase risk of pneumonia in COPD patients. Caution in patients with tuberculosis, fungal or viral infections, or untreated systemic infections. Use with caution in patients with cardiovascular disorders, diabetes, glaucoma, or thyrotoxicosis. Systemic effects of corticosteroids may occur, particularly at high doses. Monitor growth in children and adolescents. Periodic assessments of bone mineral density may be warranted with long-term use.

Contraindications

Hypersensitivity to budesonide, formoterol, or any ingredient. Not for treatment of acute asthma attacks or status asthmaticus. Avoid in patients with known or suspected quinidine hypersensitivity. Contraindicated in individuals with hypertrophic obstructive cardiomyopathy or severe tachyarrhythmias.

Possible side effects

Common: Oropharyngeal candidiasis, hoarseness, throat irritation, headache, tremor, palpitations, muscle cramps. Less common: Anxiety, sleep disturbances, cough, dysphonia, skin bruising. Rare but serious: Paradoxical bronchospasm, cardiovascular effects (tachycardia, arrhythmias), hypokalemia, hyperglycemia, adrenal suppression, reduced bone mineral density, glaucoma, cataracts, anaphylactic reactions.

Drug interaction

Avoid concomitant use with beta-blockers (may antagonize effects and cause severe bronchospasm). Use caution with CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) โ€“ may increase budesonide exposure. Diuretics or xanthine derivatives may potentiate hypokalemia. Concomitant use with other sympathomimetics may increase cardiovascular side effects. MAOIs and tricyclic antidepressants may potentiate the effect of formoterol.

Missed dose

Take the missed dose as soon as remembered, unless it is nearly time for the next dose. Do not double the dose to make up for a missed one. Resume the regular dosing schedule. Do not discontinue abruptly without medical advice.

Overdose

Overdosage may lead to symptoms such as tachycardia, arrhythmias, tremor, headache, nausea, hypokalemia, hyperglycemia, and exaggerated pharmacologic effects. Cardiac monitoring and symptomatic treatment are recommended. Consider cardioselective beta-blockers if necessary (with caution in patients with reversible airway obstruction). Corticosteroid-related effects may require supportive management.

Storage

Store at room temperature (below 30ยฐC) in a dry place. Keep the Turbuhaler in the original container with the cap tightly closed. Protect from moisture and direct sunlight. Discard 3 months after opening or when the dose counter reads โ€œ0,โ€ whichever comes first. Do not break, crush, or expose the inhaler to high humidity.
