Prilox Cream: Effective Topical Anesthetic for Rapid Pain Relief
Prilox Cream is a topical anesthetic formulation designed for rapid and effective dermal analgesia. It combines lidocaine and prilocaine as active pharmaceutical ingredients, working synergistically to block sodium channels in peripheral sensory nerves. This results in a reversible loss of sensation at the application site, making it an indispensable tool for minor dermatological procedures, needle-related interventions, and superficial skin treatments. The cream is characterized by its eutectic mixture, which allows for enhanced percutaneous absorption and predictable onset of action, providing clinicians with a reliable means of improving patient comfort and procedural outcomes.
Features
- Contains lidocaine 2.5% and prilocaine 2.5% in a eutectic oil-in-water emulsion base
- Rapid onset of anesthetic effect, typically within 60 minutes under occlusion
- Duration of analgesia persists for approximately 1β2 hours post-removal
- Water-miscible formulation that is non-greasy and easy to remove
- Available in 5g, 15g, and 30g tubes for clinical and individual use
- Standardized concentration ensuring consistent pharmacological activity
Benefits
- Provides effective surface anesthesia, reducing procedural pain and anxiety
- Minimizes need for injectable anesthetics in superficial interventions
- Enhances patient cooperation, particularly in pediatric and geriatric populations
- Reduces systemic exposure to anesthetics compared to injectable forms
- Facilitates smoother performance of minor surgical and cosmetic procedures
- Can be applied in outpatient and home settings under appropriate guidance
Common use
Prilox Cream is indicated for use as a topical anesthetic on intact skin for minor procedures such as venipuncture, intravenous cannulation, laser treatments, superficial surgical excisions, and dermatological biopsies. It is also employed for mitigating discomfort associated with cosmetic procedures including tattoo removal, chemical peels, and microdermabrasion. In clinical settings, it is frequently used prior to split-thickness skin graft harvesting and laser hair removal. Off-label uses may include management of neuropathic pain and post-herpetic neuralgia when applied under medical supervision.
Dosage and direction
Apply a thick layer of Prilox Cream (approximately 1β2g per 10cmΒ² of skin) to the target area. Cover with an occlusive dressing to enhance penetration. For adults: Apply 60 minutes prior to procedure. For children: Apply 30β60 minutes prior, depending on age and skin integrity. Maximum application area: 100cmΒ² in adults, 16cmΒ² in children weighing <10kg, and 100cmΒ² in children weighing >10kg. Maximum application time: 4 hours in adults, 1 hour in infants 1β3 months, 4 hours in infants 3β12 months. Remove cream and cleanse area before procedure.
Precautions
Use with caution in patients with impaired hepatic function, as metabolism may be reduced. Avoid application to broken or inflamed skin, as systemic absorption may increase. Do not apply near eyes or mucous membranes. Use cautiously in patients with congenital or idiopathic methemoglobinemia, or those taking oxidizing drugs. Monitor for signs of systemic toxicity in patients with large application areas or prolonged use. Not recommended for use on genital mucosa without specific medical guidance.
Contraindications
Hypersensitivity to amide-type local anesthetics or any component of the formulation. Contraindicated in patients with known methemoglobinemia. Should not be used on patients with skin conditions that may enhance systemic absorption (e.g., eczema, psoriasis, severe dermatitis). Not for ophthalmic use. Avoid in infants under 1 month of age or preterm infants due to risk of methemoglobinemia.
Possible side effect
Common: Localized pallor, redness, edema, or altered sensation at application site. Less common: Rash, itching, or burning sensation. Rare: Systemic effects including dizziness, headache, tremors, blurred vision, or tachycardia. Very rare: Methemoglobinemia (symptoms include cyanosis, fatigue, dyspnea), allergic reactions (urticaria, angioedema), or cardiovascular effects. Most local reactions are transient and resolve without intervention.
Drug interaction
May interact with Class I antiarrhythmic drugs (e.g., mexiletine) potentiating toxic effects. Concurrent use with sulfonamides, antimalarials, nitrates, or other oxidizing agents may increase risk of methemoglobinemia. CYP3A4 inhibitors (e.g., ketoconazole) may reduce metabolism of lidocaine and prilocaine. Use with other local anesthetics may have additive systemic effects.
Missed dose
Not applicable for intermittent use; Prilox Cream is applied as a single dose prior to procedures. If application is omitted or removed prematurely, reapply if sufficient time remains before the procedure, considering the required onset time.
Overdose
Systemic overdose may manifest as CNS excitation (nervousness, dizziness, tremors) followed by depression (drowsiness, unconsciousness), cardiovascular depression (bradycardia, hypotension), or methemoglobinemia. Treatment involves immediate removal of cream, supportive care, and management of symptoms. Benzodiazepines may be used for seizures; methylene blue is indicated for methemoglobinemia if levels exceed 30%.
Storage
Store at room temperature (15β30Β°C). Do not freeze. Keep tube tightly closed when not in use. Protect from light and moisture. Keep out of reach of children. Do not use beyond the expiration date printed on the packaging.

Reviews
Clinical studies demonstrate high efficacy, with >85% of patients reporting significant pain reduction during venipuncture. Dermatologists report improved patient tolerance for laser procedures. Some users note transient pallor and erythema as minor drawbacks. Overall rated as effective and reliable for indicated uses when applied correctly.
