Nasonex Nasal Spray (mometasone furoate monohydrate) is a prescription corticosteroid spray designed for the management of nasal symptoms associated with allergic rhinitis in adults and pediatric patients. It works locally in the nasal passages to reduce inflammation, providing significant relief from congestion, sneezing, and runny nose. Trusted by allergists and ENT specialists, it offers a targeted approach with a favorable safety profile for appropriate candidates.
Features
- Active ingredient: Mometasone furoate monohydrate (50 mcg per spray)
- Delivery system: Metered-dose, manual pump spray bottle
- Preservative: Benzalkonium chloride
- Available in 17g and 60g bottle sizes (approximately 60 and 120 sprays, respectively)
- pH-balanced formulation for nasal comfort
- Prescription-only medication
Benefits
- Effectively reduces nasal inflammation at the source, not just masking symptoms.
- Provides 24-hour relief from allergic rhinitis symptoms with once-daily dosing after the initial loading period.
- Minimizes systemic absorption due to its topical action, reducing risk of steroid-related side effects.
- Improves quality of life by restoring normal nasal breathing and reducing reliance on oral antihistamines.
- Suitable for seasonal and perennial allergic rhinitis, as well as nasal polyps in adults.
- Prevents recurrence of nasal polyps following surgical removal.
Common use
Nasonex is primarily indicated for the treatment of the nasal symptoms of seasonal allergic rhinitis and perennial allergic rhinitis in adults and children 2 years of age and older. It is also approved for the treatment of nasal polyps in patients 18 years of age and older. Many otolaryngologists also consider it for off-label use in cases of non-allergic rhinitis and chronic sinusitis without polyps, based on clinical judgment.
Dosage and direction
For allergic rhinitis in adults and adolescents 12 years and older: The recommended dose is 2 sprays in each nostril once daily (total 200 mcg/day). When symptoms are controlled, the dose may be reduced to 1 spray in each nostril once daily. For children 2 to 11 years of age: The recommended dose is 1 spray in each nostril once daily (total 100 mcg/day). For nasal polyps in adults: 2 sprays in each nostril twice daily (total 400 mcg/day). Shake well before use. Prime the pump before first use or if not used for 14 days or more.
Precautions
Use under medical supervision; not for ophthalmic, oral, or inhalation use. Avoid exposure to chickenpox or measles. Caution in patients with tuberculosis, untreated fungal, bacterial, or viral infections, or ocular herpes simplex. Monitor for signs of adrenal insufficiency if switching from systemic corticosteroids. Nasal septal perforation has been reported; discontinue if ulceration occurs. Use during pregnancy only if clearly needed. Not recommended during lactation without consulting a physician.
Contraindications
Hypersensitivity to mometasone furoate or any component of the formulation. Contraindicated in patients with untreated localized nasal infections. Should not be used for the relief of acute bronchospasm.
Possible side effects
Common side effects (≥1%) include headache, pharyngitis, epistaxis (blood-tinged mucus or frank bleeding), and nasal irritation. Less frequently reported: nasal burning, cough, upper respiratory tract infection. Rare but serious: anaphylaxis, angioedema, glaucoma, cataracts, adrenal suppression, growth suppression in children, perforation of the nasal septum. Report any vision changes or signs of infection to your physician.
Drug interaction
Formal drug interaction studies have not been conducted with Nasonex. However, caution is advised with concomitant use of other corticosteroids, CYP3A4 inhibitors (e.g., ketoconazole, ritonavir), or drugs that may suppress the immune system. No known interactions with oral antihistamines or decongestants.
Missed dose
If a dose is missed, use it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to catch up.
Overdose
Acute overdose is unlikely due to low systemic bioavailability. Topical corticosteroids may be absorbed in amounts sufficient to produce systemic effects. In case of suspected overdose, discontinue treatment and provide supportive care. Monitor for hypercorticism and adrenal suppression.
Storage
Store at room temperature (15–30°C or 59–86°F). Do not freeze. Keep the bottle upright and away from direct light. Keep out of reach of children. Discard after 120 sprays or as indicated on the packaging, even if bottle is not completely empty.

Reviews
Clinical studies and patient reports consistently highlight Nasonex’s efficacy in controlling nasal allergy symptoms, with many users noting significant improvement in congestion and quality of life within days. Physicians appreciate its once-daily dosing and low systemic exposure. Some users report mild nasal dryness or irritation, which often resolves with continued use. Overall, it is regarded as a first-line option for moderate to severe allergic rhinitis.
