Common use

Lquin is commonly prescribed for community-acquired pneumonia, acute bacterial sinusitis, acute bacterial exacerbation of chronic bronchitis, complicated and uncomplicated urinary tract infections, acute pyelonephritis, and skin and skin structure infections. It is also used in the treatment of inhalational anthrax (post-exposure) and plague. Its use is generally reserved for cases where first-line agents are inappropriate due to resistance, allergy, or clinical severity.

Dosage and direction

The recommended dosage varies by infection type and severity. For most infections, the typical dose is 250mg to 750mg orally once every 24 hours. Treatment duration usually ranges from 7 to 14 days, though some infections may require longer therapy. Tablets should be swallowed whole with a full glass of water, and may be taken with or without food. Patients should be advised to complete the entire course of therapy even if symptoms improve, to prevent resistance development. Dosage must be adjusted in patients with renal impairment (CrCl <50 mL/min).

Precautions

Patients should be advised to maintain adequate hydration to prevent crystal formation. Use with caution in patients with known or suspected central nervous system disorders, as fluoroquinolones may lower seizure threshold. Avoid excessive sunlight or UV exposure due to photosensitivity risk. Monitor for signs of tendon inflammation or rupture, particularly in elderly patients, those taking corticosteroids, or those with renal impairment. Blood glucose should be monitored in diabetic patients, as dysglycemia has been reported.

Contraindications

Lquin is contraindicated in patients with a history of hypersensitivity to levofloxacin, other quinolones, or any component of the formulation. It should not be used in patients with a history of tendon disorders related to fluoroquinolone use. Concomitant administration with tizanidine is contraindicated.

Possible side effect

Common adverse reactions (≥1%) include nausea, diarrhea, headache, insomnia, constipation, and dizziness. Serious but rare side effects include tendon rupture, severe hypersensitivity reactions, central nervous system effects (seizures, psychosis), and blood dyscrasias.

Drug interaction

Lquin may interact with multivalent cation-containing products (antacids, sucralfate, metal cations), which significantly reduce absorption—administer at least 2 hours before or 2 hours after Lquin. It may enhance the effects of warfarin (monitor INR closely). Concomitant use with drugs that prolong QT interval (antiarrhythmics, antipsychotics, macrolides) should be avoided. Nonsteroidal anti-inflammatory drugs may increase CNS stimulation risk. Levofloxacin may affect blood glucose levels when used with antidiabetic agents.

Missed dose

If a dose is missed, it should be taken as soon as remembered unless it is nearly time for the next dose. In that case, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one.

Overdose

Symptoms of overdose may include dizziness, tremor, nausea, vomiting, and seizures. Management is supportive; ECG monitoring is advised due to potential QT prolongation. Hemodialysis removes approximately 10% of the administered dose and may be considered in severe cases.

Storage

Store at room temperature (20–25°C or 68–77°F), in a tightly closed container, protected from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
