Levothroid (levothyroxine sodium) is a synthetic thyroid hormone replacement medication designed to treat hypothyroidism. As a bioidentical formulation of thyroxine (T4), it provides precise, consistent hormone supplementation to restore metabolic equilibrium in patients with deficient thyroid function. Manufactured under stringent quality controls, Levothroid offers reliable dosing accuracy and predictable pharmacokinetics, making it a cornerstone therapy in endocrine management. Its well-established efficacy and safety profile have made it a trusted choice for clinicians managing thyroid disorders for decades.
Features
- Contains levothyroxine sodium, USP—a synthetic T4 hormone identical to endogenous thyroxine
- Available in multiple strengths (e.g., 25 mcg, 50 mcg, 75 mcg, 88 mcg, 100 mcg, 112 mcg, 125 mcg, 137 mcg, 150 mcg, 175 mcg, 200 mcg) for precise titration
- Manufactured in cGMP-compliant facilities ensuring batch-to-batch consistency
- Tablet formulation allows for easy splitting and dose adjustment
- Minimal inactive ingredients; lactose-free formulation reduces intolerance risks
- Long shelf life with stable potency under proper storage conditions
Benefits
- Restores normal metabolic rate, alleviating symptoms like fatigue, weight gain, and cold intolerance
- Normalizes thyroid-stimulating hormone (TSH) levels with appropriate dosing
- Prevents long-term complications of untreated hypothyroidism, including cardiovascular disease and myxedema coma
- Improves cognitive function, mood stability, and overall quality of life
- Offers predictable absorption and consistent serum hormone levels when administered correctly
- Cost-effective compared to many branded alternatives with equivalent therapeutic efficacy
Common use
Levothroid is primarily indicated for the treatment of hypothyroidism, whether primary, secondary, or tertiary in origin. It is also used as suppressive therapy in the management of thyroid nodules and thyroid cancer to reduce TSH stimulation. Additionally, it may be prescribed off-label for subclinical hypothyroidism when clinical symptoms warrant intervention. Patients typically require lifelong therapy, with periodic dose adjustments based on TSH monitoring and clinical response.
Dosage and direction
Dosing is highly individualized based on patient age, weight, cardiovascular status, and severity of hypothyroidism. For otherwise healthy adults under 50, the typical starting dose is 1.6 mcg/kg body weight per day. Elderly patients or those with cardiac disease may start at 25–50 mcg daily, with gradual increments every 4–6 weeks. Administer Levothroid orally once daily, on an empty stomach, at least 30–60 minutes before breakfast. Tablets should be taken with a full glass of water; do not crush or chew. Consistency in administration timing is critical for stable serum levels.
Precautions
Periodic monitoring of TSH and free T4 levels is essential—typically 6–8 weeks after initiation or dose change, then every 6–12 months once stable. Use with caution in patients with cardiovascular disease, adrenal insufficiency, or diabetes mellitus. Pregnancy increases thyroxine requirements; monitor TSH every trimester. Avoid switching between levothyroxine products due to potential bioequivalence variations. Inform patients that certain conditions (e.g., malabsorption, bariatric surgery) may affect absorption. Do not use for weight loss in euthyroid patients.
Contraindications
Levothroid is contraindicated in patients with uncorrected adrenal insufficiency, untreated thyrotoxicosis, or acute myocardial infarction. Hypersensitivity to any component of the formulation is an absolute contraindication. It should not be used in patients with obesity without confirmed hypothyroidism, nor should it be administered concomitantly with tiratricol.
Possible side effect
Most side effects are dose-related and indicate overtreatment. Common reactions include palpitations, tachycardia, weight loss, insomnia, tremors, headache, and heat intolerance. Less frequently, patients may experience menstrual irregularities, sweating, or fever. Allergic reactions (rash, urticaria) are rare. Long-term overtreatment may lead to osteoporosis or cardiac arrhythmias. Report any chest pain, nervousness, or unusual symptoms to a healthcare provider promptly.
Drug interaction
Levothroid interacts with numerous medications. Binders like cholestyramine, colestipol, and sucralfate impair absorption—separate administration by at least 4 hours. Estrogens and selective estrogen receptor modulators may increase thyroxine requirements. Drugs like phenytoin, carbamazepine, and rifampin increase metabolism of levothyroxine. Beta-blockers may reduce conversion of T4 to T3. Iron supplements, calcium carbonate, and proton pump inhibitors significantly decrease absorption—dose separately by at least 4 hours. Anticoagulant effects of warfarin may be potentiated.
Missed dose
If a dose is missed, take it as soon as remembered the same day. If remembered the next day, skip the missed dose and resume the regular schedule. Do not double the dose to make up for a missed one. Consistent daily intake is more important than exact timing, but advise patients to maintain routine administration whenever possible.
Overdose
Symptoms of overdose resemble thyrotoxicosis: tachycardia, arrhythmias, chest pain, tremors, insomnia, headache, fever, sweating, and weight loss. In severe cases, cardiac arrest or storm may occur. Management includes discontinuing Levothroid, providing supportive care, and using beta-blockers for symptomatic relief. Consider gastric lavage if ingestion was recent. Monitor cardiac function and metabolic status closely. Serum T4 levels remain elevated for several days due to the hormone’s long half-life.
Storage
Store Levothroid tablets at controlled room temperature (20°–25°C or 68°–77°F), in a tight, light-resistant container. Keep away from moisture, heat, and direct light. Do not store in bathrooms or near sinks. Keep out of reach of children and pets. Do not use beyond the expiration date printed on the packaging.
