Dosage and direction
Dosage must be individualized based on disease severity and patient response. For asthma: The usual recommended dose for adults and adolescents (12 years and older) is 1 or 2 inhalations twice daily. The maximum recommended dose is 4 inhalations twice daily. For COPD: The usual dose is 2 inhalations twice daily. It is crucial to administer the inhaler exactly as demonstrated by a healthcare professional to ensure proper drug delivery to the lungs. Rinse the mouth with water after each use to reduce the risk of oral candidiasis. Do not exceed the prescribed dose.
Precautions
- Not for relief of acute bronchospasm; always have a fast-acting rescue inhaler (e.g., SABA) available.
- Patients should be monitored for increased use of rescue medication, as this may indicate deteriorating asthma control.
- Caution is advised in patients with cardiovascular disorders (e.g., coronary artery disease, arrhythmias), diabetes mellitus, severe hypertension, convulsive disorders, or thyrotoxicosis.
- Systemic effects of corticosteroids may occur, particularly at high doses over prolonged periods; these include adrenal suppression, growth retardation in children and adolescents, decrease in bone mineral density, cataract, and glaucoma.
- Be alert for signs of paradoxical bronchospasm; discontinue use immediately if it occurs.
- There is an increased risk of pneumonia in patients with COPD.
Contraindications
Hypersensitivity to Formoterol, Budesonide, or any other component of the formulation. This product is contraindicated for the primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
Possible side effect
Common side effects may include headache, tremor, palpitations, muscle cramps, oropharyngeal candidiasis (thrush), hoarseness, and cough. Less frequently, patients may experience tachycardia, nervousness, nausea, dizziness, and skin rash. Systemic corticosteroid effects such as adrenal suppression, reduced bone mineral density, or ocular changes (cataracts/glaucoma) may occur with long-term use at high doses. Paradoxical bronchospasm may occur in rare instances.
Drug interaction
- Concomitant use with other beta-adrenergic drugs may potentiate sympathetic effects.
- Use with caution with drugs known to prolong the QTc interval (e.g., certain antifungals, macrolide antibiotics, antipsychotics).
- CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) may increase systemic exposure to Budesonide.
- Diuretics or corticosteroids may enhance the hypokalemic effect of beta2-agonists.
- Monoamine oxidase inhibitors (MAOIs) and tricyclic antidepressants may potentiate the cardiovascular effects of Formoterol.
Missed dose
If a dose is missed, it should be taken as soon as remembered, unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one.
Overdose
Overdosage may lead to excessive beta-adrenergic stimulation and/or systemic corticosteroid effects. Symptoms may include angina, hypertension, hypotension, tachycardia, arrhythmias, nervousness, headache, tremor, muscle cramps, dry mouth, nausea, dizziness, fatigue, malaise, hypokalemia, and hyperglycemia. Cardiac arrest and even death may occur. Supportive and symptomatic treatment is indicated. Consider cardioselective beta-blockers with caution, bearing in mind the risk of inducing bronchospasm.
Storage
Store in a dry place at room temperature (15-30°C). Keep the canister away from direct heat or sunlight. Do not puncture, break, or burn the canister even after it is empty. Keep out of reach of children.
