Common use

FML Forte is commonly prescribed for short-term, intensive treatment of significant ocular inflammation. Its primary applications include the management of post-operative inflammation following intraocular surgery (e.g., phacoemulsification with IOL implantation), treatment of severe anterior uveitis, and control of robust inflammatory episodes in conditions like vernal keratoconjunctivitis, episcleritis, and certain types of keratitis. It is not typically used for mild, chronic conditions due to its potency.

Dosage and direction

The dosage of FML Forte is highly individualized based on the severity of the inflammatory condition. A typical initial regimen involves instilling 1 to 2 drops into the conjunctival sac of the affected eye(s) two to four times daily. During the initial 24 to 48 hours, dosing may be intensified (e.g., every 2 hours while awake) for severe presentations under close supervision. The frequency of application should be tapered gradually as clinical improvement is noted. Shaking the bottle well before each use is critical to ensure proper suspension of the active ingredient. Patients must be instructed on proper instillation technique to avoid contamination of the dropper tip.

Precautions

  • Intraocular Pressure (IOP) Monitoring: Prolonged use of corticosteroids can result in glaucoma, with damage to the optic nerve, defects in the visual field, and posterior subcapsular cataract formation. IOP should be checked routinely during treatment, even though FML Forte has a lower potential for this effect.
  • Prolonged Use: Extended use may suppress the host immune response and thus increase the hazard of secondary ocular infections from pathogens including viruses, fungi, and bacteria.
  • Perforation: In those diseases causing thinning of the cornea or sclera, perforation has been known to occur with the use of topical steroids.
  • Viral Infection: Use with extreme caution, and only under careful supervision, in patients with a history of herpes simplex keratitis. Corticosteroids may exacerbate the infection or mask its activity.
  • Bacterial Infection: If an infection does not respond promptly to appropriate therapy, the steroid should be discontinued until the infection has been adequately controlled.
  • Pregnancy and Lactation: Use only if the potential benefit justifies the potential risk to the fetus or infant.

Contraindications

FML Forte is contraindicated in patients with:

  • Most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
  • Mycobacterial infection of the eye.
  • Fungal diseases of the ocular structures.
  • Known hypersensitivity to any component of the formulation (fluorometholone or preservatives like benzalkonium chloride in multi-dose bottles).

Possible side effect

While not all patients experience side effects, the following have been reported:

  • Common: Transient stinging or burning upon instillation; blurred vision immediately after installation.
  • Uncommon: Elevated intraocular pressure (steroid-induced glaucoma); posterior subcapsular cataract formation with prolonged use.
  • Rare: Perforation of the globe; secondary ocular infection from pathogens including herpes simplex, fungi, and bacteria; delayed wound healing; ptosis; mydriasis; conjunctival erythema; irritation; and rarely, uveitis.
  • Systemic Effects: Although rare with topical use, systemic absorption can occur and may lead to systemic hypercorticoid effects with very prolonged use.

Drug interaction

Formal interaction studies have not been conducted with FML Forte. However, as a corticosteroid, its anti-inflammatory effects may be additive or synergistic with other systemic or topical anti-inflammatory agents. Concurrent use with other ocular medications should be separated by at least 5 to 10 minutes to prevent dilution and potential interaction. The preservative benzalkonium chloride may be absorbed by soft contact lenses; patients should be instructed to remove lenses prior to instillation and wait at least 15 minutes before reinsertion.

Missed dose

If a dose is missed, it should be instilled as soon as possible. However, if it is almost time for the next scheduled dose, the missed dose should be skipped, and the regular dosing schedule resumed. The patient should not instill a double dose to make up for the missed one.

Overdose

Topical ocular overdose is unlikely to produce life-threatening systemic effects. Acute overdose may be flushed from the eye(s) with warm tap water. Symptoms of systemic chronic corticosteroid overdose from significant absorption could include hypercorticism (Cushing’s syndrome). Treatment would involve discontinuation of the medication and supportive care.

Storage

Store at controlled room temperature 15°-30°C (59°-86°F). Protect from freezing and direct light. Keep the bottle tightly closed when not in use. Do not use if the suspension has changed color or contains precipitates. Discard the multi-dose bottle 30 days after opening. Single-use vials are for immediate use following opening and must be discarded after one application, even if medication remains.
