Common use

Doxycycline is indicated for the treatment of infections caused by susceptible strains of microorganisms, including but not limited to: upper and lower respiratory tract infections (e.g., pneumonia, bronchitis, sinusitis); sexually transmitted infections such as chlamydia, gonorrhea, and syphilis; skin and soft tissue infections; urinary tract infections; anthrax exposure; malaria prophylaxis; Lyme disease; rickettsial infections like Rocky Mountain spotted fever; and ocular infections such as trachoma. It is also used off-label for inflammatory acne and rosacea management.

Dosage and direction

Dosage is contingent upon the indication, severity of infection, and patient factors such as renal function. For most bacterial infections in adults, the typical dose is 100 mg twice daily on the first day (loading dose), followed by 100 mg once daily. For severe infections, 100 mg every 12 hours may be maintained. For gonorrhea, a single 300 mg dose may be administered. For malaria prophylaxis, the usual dose is 100 mg daily, beginning 1–2 days before travel, during travel, and for 4 weeks after leaving the endemic area. Administer with a full glass of water to reduce esophageal irritation and ulceration, and remain upright for at least 30 minutes after ingestion. To maximize absorption and minimize gastrointestinal upset, take with food or milk, though this may slightly decrease bioavailability.

Precautions

Patients should be advised to avoid excessive sun or UV light exposure due to photosensitivity reactions. Use with caution in patients with hepatic impairment. Doxycycline may cause esophageal irritation; ensure adequate fluid intake when swallowing. It is not recommended for use in pediatric patients under 8 years of age due to the risk of permanent tooth discoloration and enamel hypoplasia. May cause dizziness; caution when operating machinery or driving. Use during tooth development (last half of pregnancy, infancy, childhood up to age 8) may cause permanent discoloration of teeth. As with all antibiotics, use may result in overgrowth of non-susceptible organisms, including fungi.

Contraindications

Hypersensitivity to doxycycline, other tetracyclines, or any component of the formulation. Contraindicated in individuals with a history of severe hepatic dysfunction.

Possible side effect

Common adverse reactions include nausea, vomiting, diarrhea, dysphagia, esophageal irritation, and photosensitivity. Less frequently, glossitis, stomatitis, enterocolitis, and inflammatory lesions with monilial overgrowth in the anogenital region may occur. Rare but serious side effects include pseudotumor cerebri, hepatotoxicity, severe skin reactions such as Stevens-Johnson syndrome, and blood dyscrasias. Discoloration of teeth and inhibition of bone growth may occur if used during tooth development.

Drug interaction

Antacids containing aluminum, calcium, or magnesium; iron preparations; bismuth subsalicylate; and zinc salts can impair absorption—administer doxycycline 2 hours before or 4 hours after these products. Barbiturates, carbamazepine, and phenytoin may decrease doxycycline levels. Doxycycline may potentiate the effect of warfarin, necessitating close monitoring of prothrombin time. Concurrent use with isotretinoin should be avoided due to increased risk of pseudotumor cerebri. May reduce the efficacy of oral contraceptives; advise use of an alternative non-hormonal contraceptive method.

Missed dose

If a dose is missed, it should be taken as soon as remembered. However, if it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one.

Overdose

Symptoms of overdose may include nausea, vomiting, and diarrhea. There is no specific antidote. Management is supportive and symptomatic. Gastric lavage may be considered if ingestion was recent. Hemodialysis does not significantly enhance elimination.

Storage

Store at room temperature, 20°C to 25°C (68°F to 77°F), in a tight, light-resistant container. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
