Celebrex (celecoxib) is a prescription nonsteroidal anti-inflammatory drug (NSAID) specifically formulated as a selective cyclooxygenase-2 (COX-2) inhibitor. Common use

Celebrex is commonly prescribed for the management of signs and symptoms of osteoarthritis and rheumatoid arthritis in adults. It is also approved for the relief of acute pain, such as that following surgical procedures, and for the treatment of ankylosing spondylitis. Its use is centered on conditions where inflammation is a key driver of pain and functional impairment.

Dosage and direction

Dosage must be individualized. For osteoarthritis, the recommended dose is 200 mg per day, administered as a single dose or as 100 mg twice daily. For rheumatoid arthritis, the recommended dose is 100 mg to 200 mg twice daily. For the management of acute pain, the recommended initial dose is 400 mg, followed by an additional 200 mg dose if needed on the first day. On subsequent days, the recommended dose is 200 mg twice daily as needed. For ankylosing spondylitis, the recommended dose is 200 mg daily in a single dose or divided (100 mg twice daily). If no effect is observed after six weeks, a trial of 400 mg daily (single or divided dose) may be considered. Celebrex should be taken with a full glass of water and may be taken with or without food. The lowest effective dose for the shortest possible duration should be used.

Precautions

Patients should be advised to inform their physician if they have a history of asthma, aspirin-sensitive asthma, liver or kidney disease, hypertension, heart failure, edema, dehydration, or bleeding problems. Celebrex, like other NSAIDs, can lead to the onset of new hypertension or worsening of pre-existing hypertension. Patients should be monitored for signs and symptoms of GI ulceration and bleeding. Serious skin reactions, some fatal, can occur. Patients should be advised to discontinue Celebrex at the first appearance of a skin rash, mucosal lesions, or any other sign of hypersensitivity. Celebrex can cause serious cardiovascular thrombotic events, such as myocardial infarction and stroke. Long-term use may result in renal injury. Patients should use caution when driving or operating machinery if they experience dizziness or drowsiness.

Contraindications

Celebrex is contraindicated in patients with known hypersensitivity (e.g., anaphylactic reactions, serious skin reactions) to celecoxib. It should not be used in patients who have experienced asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs. Severe, sometimes fatal, anaphylactic reactions to NSAIDs have been reported in such patients. Celebrex is contraindicated in the setting of coronary artery bypass graft (CABG) surgery.

Possible side effect

Common side effects may include diarrhea, dyspepsia, abdominal pain, nausea, flatulence, peripheral edema, dizziness, insomnia, and rash. Serious side effects require immediate medical attention and include:

  • Cardiovascular thrombotic events (heart attack, stroke)
  • GI events (bleeding, ulceration, perforation of the stomach or intestines)
  • Hepatotoxicity (liver damage)
  • Hypertension
  • Heart failure and edema
  • Renal toxicity and kidney injury
  • Anaphylactic reactions
  • Serious skin reactions (e.g., Stevens-Johnson Syndrome, toxic epidermal necrolysis)
  • Hematologic toxicity (anemia)

Drug interaction

Celebrex can interact with numerous medications. Concomitant use with aspirin or other NSAIDs (e.g., ibuprofen, naproxen) is not recommended due to an increased risk of GI adverse events. It may diminish the antihypertensive effect of ACE inhibitors, angiotensin II receptor blockers (ARBs), and beta-blockers. Concurrent use with warfarin or similar anticoagulants can increase the risk of bleeding. Celebrex can increase lithium plasma levels. Concomitant administration with fluconazole (a strong CYP2C9 inhibitor) can significantly increase celecoxib plasma concentrations. Use with corticosteroids increases the risk of GI ulceration or bleeding.

Missed dose

If a dose is missed, it should be taken as soon as it is remembered. However, if it is almost time for the next scheduled dose, the missed dose should be skipped and the regular dosing schedule resumed. Patients should not take a double dose to make up for a missed one.

Overdose

Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, GI bleeding, hypertension, acute renal failure, respiratory depression, coma, and anaphylaxis. There is no specific antidote. Management involves supportive care and symptomatic treatment. Gastric lavage and administration of activated charcoal may be considered. Hemodialysis is not likely to be effective due to high protein binding.

Storage

Celebrex capsules should be stored at room temperature, between 20°C to 25°C (68°F to 77°F), with excursions permitted between 15°C to 30°C (59°F to 86°F). The medication must be kept in its original container, tightly closed, and out of reach of children and pets. It should be protected from moisture and light. Do not flush medications down the toilet or pour them into a drain. Unused medication should be disposed of properly via a medicine take-back program.
