Common use

Atacand is primarily prescribed for the management of hypertension, either as monotherapy or in combination with other antihypertensive agents. It is a first-line treatment option recognized by major international guidelines (e.g., ESC/ESH, ACC/AHA). In clinical practice, it is also extensively used for the treatment of chronic heart failure with reduced ejection fraction (HFrEF) to improve outcomes and reduce morbidity and mortality. For diabetic patients with proteinuric renal disease, Atacand is indicated to slow the progression of renal impairment.

Dosage and direction

Dosage must be individualized. The tablet can be taken with or without food.

Hypertension in Adults:

  • Initial Dose: 16 mg once daily.
  • Maintenance Dose: The dosage range is 8 to 32 mg once daily, depending on blood pressure response. Doses above 32 mg daily do not appear to have a greater effect.
  • Volume-Depleted Patients: A lower initial dose of 8 mg is recommended.

Heart Failure in Adults:

  • Initial Dose: 4 mg once daily.
  • Dosage Titration: Double the dose at intervals of at least 2 weeks, as tolerated by the patient, to the target maintenance dose of 32 mg once daily.

Hypertension in Pediatric Patients (1 to <17 years of age):

  • Dose is based on body weight.
  • For patients weighing >50 kg, the recommended dose is 8 to 16 mg once daily. Doses >32 mg have not been studied.
  • For patients weighing <50 kg, the recommended dose is 4 to 8 mg once daily.

Precautions

  • Pregnancy: Drugs that act directly on the renin-angiotensin system (RAS) can cause injury and death to the developing fetus. Discontinue Atacand as soon as pregnancy is detected.
  • Hypotension: Symptomatic hypotension may occur in volume- and/or salt-depleted patients (e.g., those on high-dose diuretic therapy). Correct this condition prior to administration or start therapy under close medical supervision.
  • Impaired Renal Function: Monitor renal function periodically. As a consequence of inhibiting the RAS, changes in renal function may be anticipated in susceptible individuals (e.g., patients with renal artery stenosis). Dosage adjustment may be required in patients with severe renal impairment.
  • Hyperkalemia: May occur. Risk factors include renal insufficiency, diabetes, and concomitant use of drugs that raise potassium (e.g., potassium-sparing diuretics, potassium supplements). Monitor serum potassium periodically.
  • Hepatic Impairment: In patients with severe hepatic impairment, increased systemic exposure to candesartan was observed. Initiate at a lower dose and consider slower titration.

Contraindications

  • Known hypersensitivity to candesartan cilexetil or any component of the product.
  • Concomitant use with aliskiren in patients with diabetes.

Possible side effect

Adverse reactions are often mild and transient. The most common side effects include:

  • Respiratory: Upper respiratory tract infection, pharyngitis, rhinitis.
  • Body as a whole: Back pain, fatigue.
  • Neurological: Headache, dizziness.
  • Gastrointestinal: Nausea, abdominal pain, diarrhea.
  • Laboratory abnormalities: Hyperkalemia, increased serum creatinine.
  • Cardiovascular: Hypotension (primarily in heart failure patients).

Serious but less common side effects can include renal impairment, angioedema (swelling of the face, lips, throat, and/or tongue), and fetal toxicity.

Drug interaction

  • Lithium: Reversible increases in serum lithium concentrations and toxicity have been reported during concomitant use with ARBs. Monitor lithium levels closely.
  • NSAIDs: Including selective COX-2 inhibitors, may diminish the antihypertensive effect of ARBs and increase the risk of renal impairment, especially in elderly or volume-depleted patients.
  • Potassium-Supplements and Potassium-Sparing Diuretics: Concomitant use may lead to increases in serum potassium.
  • Aliskiren: Do not use concomitantly with aliskiren in patients with diabetes. Avoid concomitant use in patients with renal impairment (GFR <60 ml/min).
  • Other Antihypertensives: Additive hypotensive effects may be seen.

Missed dose

If a dose is missed, it should be taken as soon as possible on the same day. If it is almost time for the next dose, the missed dose should be skipped, and the regular dosing schedule resumed. Do not take a double dose to make up for a missed one.

Overdose

The most likely manifestation of overdose is hypotension and dizziness. If ingestion is recent, employ supportive measures such as gastric lavage or activated charcoal. The patient should be placed in a supine position and given intravenous normal saline to restore blood volume and pressure. Candesartan is not removed by hemodialysis.

Storage

Store Atacand tablets at 25°C (77°F); excursions permitted to 15-30°C (59-86°F). Keep the bottle tightly closed to protect from moisture. Keep out of reach of children.
