Common use
Assurans (Sildenafil Citrate) is primarily indicated for the treatment of erectile dysfunction (ED) in adult men. Erectile dysfunction is defined as the persistent inability to achieve or maintain an erection sufficient for satisfactory sexual performance. Its use is predicated on the presence of sexual stimulation; it is not an aphrodisiac and does not increase libido. It is commonly prescribed for ED of various etiologies, including vasculogenic, neurogenic, and psychogenic origins, as well as ED associated with common conditions like diabetes mellitus, hypertension, and spinal cord injury. It is used on an as-needed basis prior to anticipated sexual activity.
Dosage and direction
The recommended starting dose for most patients is 50 mg taken approximately one hour before sexual activity. Based on efficacy and tolerability, the dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg. The dosing frequency should not exceed once per day. - Assurans may cause dose-related impairment of color discrimination (blue/green). Caution is advised when driving or operating machinery, especially in variable lighting conditions.
- This medication does not protect against sexually transmitted diseases, including HIV.
Contraindications
- Concomitant administration of any form of organic nitrate, either regularly or intermittently (e.g., nitroglycerin, isosorbide mononitrate/dinitrate), or nitric oxide donors. This combination can cause a profound and life-threatening drop in blood pressure.
- Concomitant use with guanylate cyclase stimulators, such as riociguat.
- Known hypersensitivity to Sildenafil Citrate or any of the excipients in the Assurans formulation.
- Patients with a history of non-arteritic anterior ischemic optic neuropathy (NAION), which has been reported rarely post-marketing in association with PDE5 inhibitor use.
Possible side effect
Like all medicines, Assurans can cause side effects, although not everybody gets them. Most are mild to moderate and transient.
- Very common (≥1/10): Headache, flushing.
- Common (≥1/100 to <1/10): Dyspepsia, nasal congestion, dizziness, visual disturbances (including blurred vision, cyanopsia), nausea.
- Uncommon (≥1/1,000 to <1/100): Hypotension, syncope, tachycardia, palpitations, vomiting, rash, hematuria, eye pain, photophobia, lacrimation increase.
- Rare (≥1/10,000 to <1/1,000): Priapism (a prolonged and painful erection lasting more than 4 hours; requires immediate medical attention to prevent permanent tissue damage), hypersensitivity reactions, seizure, transient global amnesia.
- Frequency not known: Non-arteritic anterior ischemic optic neuropathy (NAION), sudden hearing loss, retinal vascular occlusion.
Drug interaction
- Nitrates: Absolute contraindication. Concomitant use can lead to severe hypotension, syncope, myocardial infarction, or stroke.
- Alpha-blockers (e.g., doxazosin, tamsulosin): Concomitant use can potentiate the blood-pressure-lowering effects. Assurans should only be initiated in patients on stable alpha-blocker therapy, and the lowest dose of Assurans should be considered.
- Potent CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir, clarithromycin): These drugs can significantly increase Sildenafil plasma concentrations. A maximum dose of 25 mg of Assurans within a 48-hour period is recommended.
- Other antihypertensives: Additive hypotensive effects are possible.
- Alcohol: Excessive consumption can increase the risk of orthostatic hypotension and dizziness.
Missed dose
Assurans is not intended for chronic daily dosing but is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. Simply take the tablet approximately one hour before your next anticipated sexual activity. Do not take a double dose to make up for a missed one.
Overdose
In cases of overdose, the expected clinical effects would be an extension of the drug’s known adverse effects, primarily severe hypotension and syncope. Standard supportive measures should be employed, including keeping the patient in the supine position with the legs elevated to promote venous return. Hemodialysis is not expected to accelerate clearance, as Sildenafil is highly protein-bound and extensively metabolized.
Storage
Store below 30°C (86°F). Keep the tablets in their original blister packaging to protect from light and moisture. Keep out of the sight and reach of children. Do not use after the expiration date printed on the packaging.
